Registration Process

Please read below. These steps will make your registration process go smoothly:

1. Please fill out all the sections of the registration form completely. Fields marked with an asterisk (*) are required.
2. Athletic Packet – It is the player’s responsibility to turn in their athletic packet to the attendance office. If he is coming from another sport, it is mandatory that they turn in the Emergency Contact Card page only.
3. Volunteers – We appreciate your involvement with your son’s lacrosse program. The team belongs to all of us – get involved to make it the best experience for all!
4. Payment – You can pay by credit card via PayPal (PayPal account is NOT required), with your PayPal account or by check. If paying by check, please be prompt in sending your payment.

5. Registration Donation - Your tax deductible donation goes directly to your son’s lacrosse program. It may cover clinics, team dinners, tournaments, game jerseys, trainer overtime, player awards, equipment, recruiting seminars, weight & conditioning instruction, filming, and scholarships, so every interested player is able to participate.
6. Sponsor - If your company is interested or you know anyone that might be interested in becoming a sponsor, please fill out the form in the sponsorship section on the website.
7. MBAF Player Participation Cost - $325 contribution that is paid to the MBAF. This fee covers transportation, coaches stipends, safety equipment and sports trainers. If $325 was not submitted with your athletic packet, please visit www.mbathleticfoundation.com to pay online.
